The privacy wall is the point. The minimized report carries only what is necessary — who participated, and a de-identified aggregate trend. It is mandated by the CMQ Code of ethics (M-9, r.17, art. 67) and Quebec's Law 25 / PIPEDA, and it's also the point: scale with a physician's signature, without ever handing an employer protected health information.

Questions employers ask

Is this legal across Canada?

Yes. The Canada Health Act's extra-billing and user-charge rules apply only to insured medically-necessary services. Everything we sell — the annual bilan de santé (preventive screening) — is an employer-commissioned, uninsured, third-party service, billable employer-to-vendor in every province. It's the same lane the national occupational-health firms operate in. If anything ever surfaces a medically-necessary need, that component is routed to the public plan, never billed to you.

Does the employer ever see medical details?

No. A licensed physician sits between the clinical file and you. You receive only participation (who completed their bilan) and a de-identified, aggregate population picture. Never the diagnosis, symptoms, medications, or anyone's individual record — mandated by the CMQ Code of ethics and Quebec's Law 25 / PIPEDA.

Who makes the medical decisions?

A board-certified physician — always. Our system organizes the intake, history and records (facts only — it never diagnoses, triages or risk-scores); the physician applies judgment, makes every determination, and signs it. That keeps the service off the regulated medical-device line and puts a real physician's name behind every call.

How does the Bilan de santé work — and what does the fee cover?

For each enrolled employee, by age and sex, we produce the screenings Santé Canada recommends — mammography, colorectal (FIT / colonoscopy), diabetes, depression, cervical, lipids and more — with physician-signed requisitions. The employee does the tests free in the public system (CLSC / hospital). When results return, the physician explains them over Spruce, email or SMS — "all good", or "your mammogram is abnormal, we'll arrange an ultrasound / a surgeon / a repeat in 5 months" — and arranges any work-up through RAMQ. The yearly fee covers the plan, the requisitions, and that physician follow-up; the tests themselves stay publicly covered.

Is this a tax-deductible expense for the company?

Generally yes — employer-paid preventive and occupational health is an eligible business expense, and we provide a receipt. Whether it counts as a taxable benefit to the employee depends on how it's structured (for example, as a Private Health Services Plan); confirm the treatment with your accountant.

What if an employee never logs in or does the tests?

The bilan is billed per enrolled employee for the year — like a benefit premium. It's non-refundable and doesn't carry over to next year, so if someone chooses not to participate, the seat isn't refunded. We help you drive uptake with reminders and the aggregate participation dashboard.

How does payment and document upload work?

You pay first, by headcount ($50 per employee), then a secure, payment-gated page unlocks where you add your employee roster (names + emails) so we can invite each person to Spruce. Everything is handled under signed agreement, encrypted, and processed with Canadian data residency (ca-central-1) for Quebec and enterprise clients.
